Risk assessments and care plans for vulnerable people living at home? |
does anyone know is such plans exist and if so where can I ger them please risk assessments and care plans are put into place for everyone that is cared for! anybody that is move or handled it is a legal requirement. it is written and discussed with everyone involved. it has to be done by a proffessional i.e. social services, occupational therapist, physiotherapist or person within the care company that is qualified to do so not just anyone!!they have to be trained to do these. when a risk assessment is done they look at five different areas (T.I.L.E.E.) TASK, INDIVIDUAL(the workers)LOAD(the person or object) ENVIRONMENT,AND EQUIPMENT you just walk around the place & make a written note of everything that could be a possible hazzard to somebody...this is risk assessment. Care plans i dont know but the risk assessment is for you to list the hazards etc that can cause harm and then you find away to put control measures in place to reduce this or even eliminate the risk completely. A mental health practitioner or social worker will normally do a tailored assessment for their client if the client is deemed to be in need. My mom does this. She is a senior carer for the old and disabled. They are usually done before caring can start on a new client and every 6-12 months by a senior carer. all risk assessments and care plans are carried out in the community with home care managers, social workers district nurses mental health teams physios and occupational therapists |
