Risk assessments and care plans for vulnerable people living at home?


does anyone know is such plans exist and if so where can I ger them please

risk assessments and care plans are put into place for everyone that is cared for! anybody that is move or handled it is a legal requirement. it is written and discussed with everyone involved. it has to be done by a proffessional i.e. social services, occupational therapist, physiotherapist or person within the care company that is qualified to do so not just anyone!!they have to be trained to do these. when a risk assessment is done they look at five different areas (T.I.L.E.E.) TASK, INDIVIDUAL(the workers)LOAD(the person or object) ENVIRONMENT,AND EQUIPMENT
they may also include any behaviours or reasons for behaviour if relevant/medication also. the idea is to reduce the risks of the hazards identified to what the law says is a minimum level reasonably practicable.N.B. note it says reduce it does not say eliminate.and any control measures can then be included so that all moves are safe for both the person being assisted /and the person doing the work.and then this should be available for all to read before anything is done for this person(all people NEEDa individual risk assessment but you can also have a generic risk assessment for tasks that can be grouped together i.e working with people in a specific bathroom etc.

you just walk around the place & make a written note of everything that could be a possible hazzard to somebody...this is risk assessment.

i am doing this at work at the moment...everything has to be written down & kept in a book! for example..we have a bacon slicing machine..i have to make a note of who uses it...does it have a safety guard & has staff been instructed on proper use...that type of thing.

your local enviromental health will have booklets on this subject & they are very good...if you need help or advice..they will come out & talk to you.

Care plans i dont know but the risk assessment is for you to list the hazards etc that can cause harm and then you find away to put control measures in place to reduce this or even eliminate the risk completely.

E.G. Manual handling,

Control measures i.e 2 man lift, hoist, competent and trained persons.

You can carry out this yourself if you feel confident enough.

A mental health practitioner or social worker will normally do a tailored assessment for their client if the client is deemed to be in need.

If you have concerns about someone, you could ask for them to be referred by their GP.

My mom does this. She is a senior carer for the old and disabled. They are usually done before caring can start on a new client and every 6-12 months by a senior carer.

all risk assessments and care plans are carried out in the community with home care managers, social workers district nurses mental health teams physios and occupational therapists
