I need a form to assess a residents ability to use the nurse call light?

I work at a nursing facility and the state wants us to have a form to assess our residents ability to use the nurse call light system,i need it ASAP

i do not know about a form but after working in nursing homes and hospitals for 39 years
we accessed like this
does a person have use of their hands
we had two alternate ways of supplying a call bell other than a standard push button
we used a flat cushion type left to the side of the head that the patient could turn their head to push against and with one M.S, patient a call bell was adapted where a connection with a mouthpiece was placed to the side of her head and she could blow into it to light the bell
maybe you can go from there to create a form

check with another facility. ask what kind of form they use. you could use it as a guide. The state office may have one you can adapt also. have copies faxed or e-mailed to you.
