Im doing a nursing assignment...(charting)?

What are the legal requirements for accepting charting thats done in the hospital?

This includes spelling and other errors? what must be included in every documentation entry, the types of charting frequently performed by nusing assistants?

for CNA in a hosptial
is uallay the vital signs and thing they may say, or things you see
like a bleeding tv site etc
ambulate/ or use bed pan, the color etc or what ever is in it
don't want to gross anyone out you know lol
weights etc.
charting for CNA are not difficut at all, the spelling is more important for the RN etc
and most hospitals have gone to online charting
so spelling is not an issue
you just check off what you did etc

I am not sure what kind of charting you are asking. I am an RN and work in a hospital, and I am not aware of any charting that the CNA's must do. The CNA's take vital signs, I/O's, accu checks, but it is the nurses responsibility to review these before she charts/documents in the patient's record.

CNA's sometimes record vital signs in the chart, and I guess might chart if they give a patient his food, but that's essentially it; it would, however, depend on the hospital's policy....
