renewal of my nurse aide certificate I answered this already but some one gave a long drawn out answer so mine ended up at the bottom. Here goes again. Contact your local state agency , department of humnan services and they can give you the number. For evevry state the form is somewhat different. Just ask them to mail you a form to renew it.
