Which action by the new graduate indicates a need for further education about pressure ulcer care?
a. The new graduate uses a hydrocolloid dressing (DuoDerm) to cover the ulcer.
b. The new graduate inserts a sterile cotton-tipped applicator into the pressure ulcer.
c. The new graduate irrigates the pressure ulcer with a 30-ml syringe using sterile saline.
d. The new graduate cleans the ulcer with a sterile dressing soaked in half-strength peroxide.

D. Using peroxide is not appropriate and no longer acceptable way of caring wounds as per latest evidence. So, it does require a further education.
Using hydrogen peroxide on the wound is no longer a practice as it disturbs the epithelial migration within the wound during the healing process. It does also affect the granulation.

i am an RN as well. In our hospital we would need an order to debride.

I would say either A or D would be the correct answer. At my work we generally wouldn't put a Duoderm dressing on a stage 3. Usually when doing a stage 3 dressing you would use 0.9% Normal Saline wet to dry dressing. Sometimes you could use Accuzyme ( or a debriding agent). Hope this helps!
