Extemporaneous preparation of unit dose medications must have?


A. a special spot on floor stock.
B. a recording sheet with specific drug, date, and lot number information.
C. multiple dose labeling.
D. the sign-out initials of nurses who administer medications to patients

Extemporaneous meds are oral meds that are compounded and not given as a whole tablet for example. Answer C is definitely incorrect because extemporaneous meds are not IV meds (multidose labels are for multidose vials of IV meds).

Answers A and B are incorrect -- it is not necessary, for example, to have a special spot on floor stock, or record the lot number of an adult-strength po med from which oral extemporaneous liquids are often prepared for children.

The only answer that is right (for all meds -- remember the 5 rights -- that's what the sign-out initials are for) is D.
