For nurses/nursing students...I need information on NG tubes.?


I need detailed instructions for nasogastric lavage and gavage. Unfortunately my textbooks are seriously lacking. Are there any websites or can you provide some information? This is for a clinical skills check-off.

tru You tube. You would be amazed what you can find on there. I did a search for foley catheters and found 2 videos..one of a RN instructor inserting one on a manequin and another of a video of an actual patient.

Inserting a Nasogastric Tube
Preparation:

Review procedure 鈥淚nserting a Nasogastric Tube鈥?in Fundamentals of Nursing.
Review the following videos in Delmar online, 鈥淚nserting and Maintaining a Nasogastric Tube鈥?

1. Gather supplies: nasogastric tube size (adult: 14-18 Fr, infant/child: 10-14 Fr) ,lubricant, clean gloves, stethoscope, tape, safety pin, irrigation kit, and glass of water if client able to take fluids.



2. *Wash hands.



3. *Identify client and explain procedure.



4. Raise client to high fowlers position and place towel over chest. Prepare tape to mark tubing and secure to client鈥檚 nose.



5. Put of clean gloves and determine length of nasogastric tube to be inserted by measuring tube from tip of nose to tip of the ear to the xyphoid process. Mark tube with tape or note the black marking on tube.



6. Lubricate tip of tube and insert into nostril advancing towards posterior pharynx.



7. Once tube in the posterior pharynx have client tilt head forward while supporting back of client鈥檚 head to maintain position.



8. Ask client to swallow or if able take a drink of water. As client swallows advance the tube until swallowing complete. Repeat swallowing until tube is at the desired level



9. Temporarily tape tube to nose while you assess proper tube placement.



10. *Assess tube placement using two methods. Auscultate with stethoscope over left upper abdominal area while injecting 10-20 ml of air into tube and noting air sound in gastric region (left upper quadrant). Aspirate gastric contents, yellow to green colored fluid.



11. Secure tube by taping to bridge of nose and anchoring to client鈥檚 gown with tape and safety pin.

12. Connect tube to suction and set at ordered level.



13. If tube has an air vent, (short blue piggyback tube), instill 10-20 ml of air to ensure patency. Maintain air vent above gastric region to prevent gastric drainage exiting tube. If ordered may use reflux valve at the end of air vent.



14. Dispose of supplies and position client for comfort.





15. *Wash hands.





16. Document procedure.
