I would like to get an Example of the Nursing Process in Action, PLEASE!!!!?


Can someone give me an example of the nursing process in action, I would like it to be simple and strait-forward. THANK YOU!!!

Thanks Mago for your answer, but can you give an easier case, because I want to present it to fisrt year students.

The nursing Process
1. Assessment (head to toe or focused assessment)
2. Data (objective and subjective info ex. diminished lung sounds and pt complains of shortness of breath)
3.Planning(set a goal for your patient ex patient will maintain oxygen saturation of 95%)
4. Implementation (do the things that will make your goal happen ex have your patient turn cough deep breath, supplemental oxygen)
5. Evaluation(did my patient maintain oxygen saturation of 95%)

I hope this is what you were looking for

1. assessment -
pt has stage 1 decubitus on coccyx.
pt c/o "soreness" in coccyx region.
pt is ordered on bedrest.
2. diagnosis -
impaired skin integrity related to immobility
3. plan/implementation -
turn q 2 hours.
back rub, lotion to bony prominences q 2 hours.
assist with ROM q 2 hours.
referral to wound care.
4. evaluation -
less redness on coccyx.
no new areas of skin breakdown.
pt c/o less pain in coccyx region.

edit....impaired skin integrity IS one of the easiest, and most common, case any nurse will encounter...you could change the stage 1 decub to just reddened skin....turning a pt confined to bed, giving a back rub, and assessing for skin breakdown is something even a nursing assisstant does in hospital...and most of our assisstants are nursing students!
