What are the responsibilities of a private duty nurse?

The care of her patient is her sole duty. (hygiene, meds, changing bedding, etc)

First, I need to clarify, are we speaking of a nursing assistant/aide or a registered RN or LPN.

If we are talking about a nursing assistant/aide, which I'm more familiar with, they are responsible for the client's well being. They perform direct care services such as bathing, dressing, assisting in/out of bed, minor exercise as prescribed by a physical therapist. If the client is unable to prepare their own food, cooking and general cleaning, laundry if needed, only in the areas used by the client. They are not maids, cooks, babysitters. Their only focus is on the day to day requirements of the client.
If a registered nurse, provides in-house medical support, works closely with the client's physician in monitoring the client's health, less personal care but will provide bathing, assistance dressing, and make the bed. Follows the plan of care prescribed by the client's physician, administers medication, injections if required.
