I need a doctors help...?

i am a nursing student and first tym to have a case study....i need to research on a pt with DM - nephropathy, NIDDM, and HPN the medical managements rationale behind the orders of the doctors

I need to know the rationale of the following medical orders of the doctor coz i dunno.... hope u could help me....

IVF D5W 1L x KVD
D50- 50
RBS stat Na, K, BUN, creatinine
12 Lead ECG
Chest X-ray PA lateral

Treatments:
CBG monitoring every 6 hours pre meal and at midnight
Burinex
Monitor urine output and input every 4 hours
O2 inhalation 2-3 L/min via NC
Catheter now then MIO every shift
RRS after 1 hr
IVF D5W1L
+ Catapres 150 mg/ tab BID

Forgram 1gm IV q 12hr ANST
Zithromax 500mg 1 tab; OD
IVF D5NSS1L x 15gtts/min
increase IVF rate to 20 gtts /min
Buscopan 1 amp, IV

Hmmm...seems a bit advanced for your very first case study.

Here's a bit of help (you really should do your own homework, it is the only way you will learn!)

First...do you know the medical terms you listed?

nephropathy: a disease dealing with the kidneys;
NIDDM: a diabetic who does not use insulin to control their blood glucose levels
HPN: hypertension: high blood pressure

Obviously, the biggest problem this pt has is with his kidneys. Burinex is a loop diuretic...gets rid of excess fluid by acting on the kidney; The forgram & zithromax are both antibiotics; Buscopan is a pain and spasm reliever used for abdominal pain. Obviously, the catheter keeps urine from accumulating in the bladder; Lab tests for Na , K+, BUN, and creat. are all to check function of the kidney (esp. BUN and creatinine). The measuring of urine output gives an idea how well your treatment is working. The catapres is for the high bp, which is always made worse by kidney problems.

Obviously, you see that the doc is monitoring the blood glucose level of the pt carefully, since they are NIDDM.
