A nures assess a pressure sore, what would the plan be for the care of the wound?

Pressure sores are difficult to avoid if a person is bed-ridden. My husband got some pressure sores toward the end of his life. The most important thing is to relieve the pressure, so make sure the patient is moved more often. There is a good product now to cover the sore so that it is protected which can be really helpful. there are different stages of pressure sores..so the treatment would be different depending on the stage...Although it may take some time, most stage I and stage II sores will heal with conservative measures. But stage III and stage IV wounds, which are less likely to resolve on their own, may require surgery.

The first step in treating a sore at any stage is relieving the pressure that caused it.

The care depends upon the stage of the pressure sore, meaning the depth of the break down. They range from Stage 1 where the skin is just pink from friction to Stage 2 where a blister might be formed and so on.

Keeping the patient off of the area in question is of utmost importance. If there is drainage, it is important to keep it off the surrounding skin.

Doctors usually prescribe the regimen for decubitus ulcers.

why have you been sitting on your a--e too much. keep changing position as for treatment it all depends how bad it is.if its red and sore just put on some antiseptic cream on it and give it a light rub to get the circulation back.if the skin is broken do much the same.if its worse than that go see your doc.
